Extension recomendation - Joomla! Forum - community, help and support

hi, wondering if of fine people able suggest extension following scenario:

i wish design form member can submit , able query input database , data outputted in table form, example:
submission page

name: [text box]
type: [predefined text drop down box]
reason: [predefined text drop down box]
start date:[standard date selector]
end date:[standard date selector]
submitted by:[predefined text drop down box]
image upload:[file upload box]

on page

search name: [search box submitted data]
then once submit pressed list of results shown

on page

output text of details ready entered (in table if possible) , uploads listed via hyperlink or thumbnail if possible

i know asking lots 1 extension save me lot of time if had use 1 or 2 apps achieve this

there's chrono forms, it's kind of technical , may require bit of expertise both front end gui use bit of tech saavyness backend php/db stuff can incorporate via front end gui of chrono forms.

you can create multiple forms , embed them in articles, html modules, link them directly. easy build forms , setup.
